What Is Post-Hospital Discharge Care?
Post-hospital discharge care — sometimes called transitional care or hospital-to-home care — provides professional support during the critical recovery period after a hospital stay. This might follow surgery, a fall, a stroke, a cardiac event, or any other acute illness or injury that required hospitalisation.
The first weeks after a hospital stay are often the hardest to manage alone. Having attentive support at home can make that time feel far less overwhelming for the whole family.
Who Needs Post-Hospital Discharge Care?
Post-hospital care is appropriate for any older adult who is returning home after a hospital stay and who does not have family members available to provide full-time support during recovery. It is particularly important for people who live alone, those who have had major surgery (such as hip or knee replacement), and those recovering from strokes, falls, or cardiac events.
Even when family members are present, the demands of post-hospital care can be overwhelming — particularly if the person requires assistance with personal care, complex medication regimens, or mobility. Professional support allows family members to be present as loved ones rather than as carers, which is better for everyone.

They watch for changes such as new pain, confusion or wound problems, and report them promptly to the family or health professionals. Clinical tasks are done by our enrolled and registered nurses.

Our services are available to private self-funded clients and NDIS self- and plan-managed participants. Support at Home clients may be able to access Serenity’s services through their registered provider where an approved service arrangement is in place.
